The protocol

Cool first. Transport second.

It is the single most counter-intuitive rule in sports medicine, and the one that saves lives. An athlete in exertional heat stroke needs core temperature down before they go anywhere. Every minute in the ambulance instead of the tub is a minute the brain and organs stay above the danger line.

Recognise

Core temperature above 105°F with central nervous system change — confusion, aggression, stumbling, collapse. Not every athlete stops sweating. Do not wait for a textbook case.

Immerse

Whole-body cold water immersion, on site, immediately. Tub, stock tank, tarp — whatever is ready before the season starts. Aggressive cooling beats every other intervention.

Then transport

Cool to roughly 102°F before the athlete leaves the field. Your EAP names who calls, who cools, who meets EMS at the gate — and you have rehearsed it.

Every coach on your sideline should be able to do this from memory. That is the whole point of the certification — not paperwork, but muscle memory on the worst day of your career.
